Severe disease and intensive care unit (ICU) admission occurred more often among COVID-19 than influenza patients (26 [93%] vs. 19 [58%], p=.003 and 8 [29%] vs. 2 [6%], p=.034). COVID-19 patients were hospitalized longer than influenza patients (six days [IQR 4-21] vs. 3 [2-4], p<.001).

Most of the patients with COVID-19 had abnormal chest radiology (84%, p < 0.001) in comparison to influenza type A (57%, p < 0.001) and B (33%, p < 0.001). The incubation period in COVID-19 (6.4 days estimated) was longer than influenza type A (3.4 days). Likewise, the duration of hospitalization in COVID-19 patients (14 days) was longer than influenza type A (6.5 days) and influenza type B (6.7 days).

For MERS and SARS, low viral shedding and high proportions of symptomatic cases in the early stage of infection, and lack of transmission by pre‐symptomatic and asymptomatic infection enable the symptomatic identification, isolation and quarantine of the transmission sources. Therefore, syndromic surveillance, isolation of patients and quarantine of their contacts were effective measures to contain the spread of SARS‐CoV and MERS‐CoV.

The role of asymptomatic transmission in H1N1pdm09 spread is not categorical.

All ages of people were susceptible to these 3 CoVs, but senior people were more likely to be infected with SARS-CoV-2 and develop more severe clinical symptoms, while mostly younger people were infected with SARS-CoV.

While the range of symptoms for the two viruses is similar, the fraction with severe disease appears to be different. For COVID-19, data to date suggest that 80% of infections are mild or asymptomatic, 15% are severe infection, requiring oxygen and 5% are critical infections, requiring ventilation. These fractions of severe and critical infection would be higher than what is observed for influenza infection.
