Isabela will leave at the end of her personal quest "To Catch A Thief" near the end of Act 2, and will return at the END of Act 2 if you have completed her relationship midpoint quest "Questioning Beliefs". This requires you to reach either 50% friendship *or* 50% rivalry, then speak to her at the Hanged Man. You must reach the midpoint relationship before completing "To Catch A Thief". If you do not do this, she may not return. Some people have reported that she returns even without completing Questioning Beliefs. Mary Kirby, one of the Bioware developers, has said that you must complete Questioning Beliefs to get her to come back.
